Complete Screening Questionnaire



A screening questionnaire for autism is a short set of structured questions designed to identify whether someone may show characteristics associated with Autism Spectrum Disorder (ASD) or ADHD and whether a full diagnostic assessment should be considered.

Screening questionnaires do not provide a diagnosis. They are an important first step in understanding needs and guiding appropriate next steps.

We will invite you to a face-to-face initial assessment at our practice in Westhoughton, Bolton.

Here we take a detailed history and determine the best path for you or your child.  

We will share a clinical formulation with you and work with you to decide on the next steps of your journey.

The cost of an initial assessment with report is £500.

Your onward journey



Depending on the results from the Initial Assessment, we will be able to advise you on the best course of assessment of treatment for your child or young person.

• If we think an Autism assessment or ADHD assessment is required, we’ll discuss with you at this point.

• If we feel that a full autism or ADHD assessment would not be beneficial, we will discuss other support and treatments which could be beneficial to your child or young person.
